MyChart Features

  • Send messages to select members of your care team for non-urgent medical advice
  • Complete the Pre-Check-In process up to two days prior to an upcoming appointment (saves times at the welcome kiosk when you come into the office!)
  • Request a future appointment
  • Request a medication refill
  • View your test/lab results, provider notes and appointment summaries
  • View and update your health issues, immunizations, prescriptions, and allergies
  • Receive preventive care reminders and questionnaires from your provider
  • Opt in for paperless billing

Signing Up for MyChart

Established Signature Health patients have three options to activate your MyChart account:

  1. Complete and submit the online form here (if you are not an established patient, please call 440-578-8200 to establish care).
  2. Ask a member of your care team.
  3. Call 440-578-8217 and ask to activate MyChart.

When you receive the activation email or text, follow the prompts to set up a user name and password. Accept the terms and conditions, and you will be ready to explore your personal MyChart portal.

Logging in to MyChart

Once your account is active, you can log in 24/7.
A free MyChart mobile app also is available on the App Store or Google Play.
